The patient can choose to stop taking alcohol or continue to drink after completing alcohol treatment. The medicine has been considered helpful in treating anxiety and depression in individuals suffering from alcoholism or substance abuse. Studies have shown the success of Baclofen for anxiety in lowering the anxiety level in drug addicts.

Baclofen Abuse

Baclofen medication belongs to the group of gamma-aminobutyric acid (GABA) agonists, which are naturally occurring neurotransmitters in the brain. Based on the FDA label information, the precise Baclofen mechanism of action is not fully known. The medicine works by blocking the activity of nerves in the brain. When released in higher quantities, GABA can lead to issues of coordination and confusion in reflex actions. 1. It is also used in some drug rehabs to treat conditions of mental health caused by alcohol addiction and substance abuse.
2. Four studies did not report numbers and reasons of dropouts or missing data for each group, so we judged them at unclear risk of bias (Krupitskii 2017; Leggio 2015; Mishra 2010; Morley 2014‐LD; Morley 2014‐MD).

These positive findings were then supported by retrospective studies (51, 55, 64, 65) and by one recent RCT (41), while another RCT in AUD patients with liver impairment did not report differences between baclofen and placebo (36), as detailed above. Baclofen treatment should be avoided in patients with liver cirrhosis complicated by encephalopathy (120) or administered at lower doses (e.g., 15 mg/day) among patients with hepatorenal syndrome (121). However, patients with these severe disorders rarely require pharmacological treatment to reduce or stop alcohol consumption given their already serious clinical condition. However, another systematic review found that baclofen may increase the rate of abstinent days (Agabio 2021b). As this review was aimed at investigating the potential role of anxiety in influencing the effects of baclofen, it included only RCTs that provided anxiety levels of participants both at baseline and at the end of treatment. This review found that baclofen increases the rate of abstinent days only among participants with high anxiety levels and not among participants with low anxiety levels at baseline (Agabio 2021b).

Benefits of Professional Addiction Treatment

Some wonder if this approach to alcohol and drug use treatment is the same as “trading one addiction for another”. It is of interest that women who received 90 mg/day baclofen had poor tolerability with 58% dropping out or reducing dose primarily related to sedation/drowsiness. This is consistent with reports that women have more adverse effects from baclofen than men [31]. However, in the current trial, tolerability in men was quite good even at 90 mg/day. While limited, our urine baclofen data did not indicate that women had higher levels of baclofen than men suggesting that the observed increase in CNS effects of baclofen in women may be a pharmacodynamic effect.

Four hundred and thirty individuals were screened by phone between December, 2013 and June, 2017 (15 excluded for drug use disorder) to yield 153 participants who were screened in-person to yield 120 randomized participants (see Fig. 1, CONSORT). When Baclofen is injected, it can create withdrawal symptoms if not received in a timely manner. Characteristics of studies awaiting classification [ordered by study ID]

The percentage of days abstinent at the of treatment was 55.4% with baclofen and 33.4% with placebo. We identified five studies with 594 participants reporting muscle pain. There was no difference between baclofen and placebo (RR 0.93, 95% CI 0.38 to 2.25; Analysis 1.17). There was no difference between baclofen and placebo (SMD −0.01, 95% CI −0.14 to 0.11; Analysis 1.9). The studies administered baclofen in daily doses ranging from 30 mg to 300 mg.

Baclofen was initially shown to have efficacy in alcohol use disorders in a small trial in Italy [26]. Since then, ~1500 subjects have been studied in placebo-controlled clinical trials primarily in Europe with several meta-analyses published [7,8,9]. First, is that published trials have been mixed with some reporting no benefit to baclofen and others showing efficacy on total abstinence and time to lapse [7]. These mixed findings are not unusual in the alcohol use disorder field, e.g., two large U.S. trials [29, 30] failed to show efficacy of acamprosate though the FDA-approved acamprosate based on European trial data. With regards to moderators, we find evidence for an effect of sex but not for prior level of alcohol consumption on response to baclofen.

While numerous case reports, case series and open‐label studies have been published indicating the effectiveness of baclofen in people with AUD, randomised controlled trials (RCTs) conducted to date have yielded conflicting results (for an overview see de Beaurepaire 2019). The diversity of findings has stimulated the discussion on potential moderators of effectiveness (Agabio 2018a; de Beaurepaire 2019; Leggio 2010; Morley 2021). A series of case reports found that the administration of baclofen markedly reduced alcohol intake in people with AUD (Agabio signs of being roofied 2007; Ameisen 2005; Bucknam 2006; Flannery 2004; Krupitsky 1993; Pastor 2012). Ameisen 2005 described the personal history of a French physician, who conducted an original dose‐finding curve with baclofen with the aim of treating his own AUD (Agabio 2014; de Beaurepaire 2019). He tested oral baclofen, starting with the ‘conventional’ dose of 30 mg a day and increased the daily dose up to 270 mg after five weeks, experiencing, « complete medication‐induced suppression of craving » for alcohol with alleviation of comorbid anxiety (Ameisen 2005).

Baclofen lowers the seizure threshold and may precipitate seizures in people with a history of epilepsy. Therefore, it is essential alpha-pyrrolidinopentiophenone function to evaluate possible vulnerability to seizures. Epilepsy is a contraindication for the use of baclofen in some countries.

Baclofen likely reduces the risk of relapse to any drinking and increases the percentage of abstinent days, mainly among detoxified participants. It does not increase the number of participants with at least one adverse event, those who dropout is dmt addicting for any reason or due to adverse events. It probably does not reduce number of heavy drinking days and the number of drinks per drinking days. Current evidence suggests that baclofen may help people with AUD in maintaining abstinence.
